I did not send the Cover Sheet, at first. So the court was returning me my Summons form And Sheriff’s check at the same time that I was indeed sending the belated cover sheet. No need to panic. The court’s online access shows them adding docs as they arrive.
Today I returned to them my Summons form and $40 Sheriff’s Dept check (Summons Service) with a copy (printed from the online court docs) of the recorded Cover Sheet & Complaint. Many forms to come.
